About the Job
Adecco Healthcare is seeking a dedicated RN Case Manager to join our growing team in Columbia, SC. This role is perfect for a compassionate and experienced nurse who is passionate about providing care to patients in hospice and home health settings. The ideal candidate will have a strong background in nursing, a team-oriented attitude, and a desire to make a meaningful impact in their community. This is a direct-hire, full-time, and permanent.
**Compensation and Benefits:**
- Hourly pay: $39 - $41 per hour.
- Mileage reimbursement at 62.5 cents per mile.
- Paid training, with the first week requiring travel to Charlotte and Fort Mills for clinical classes.
- Training is typically completed within 1 to 1.5 months, after which you will work independently.
**Key Responsibilities:**
- Provide compassionate care to patients in hospice and home health environments, conducting 4-5 patient visits per day.
- Develop and implement patient care plans, working closely with patients, their families, and a multidisciplinary healthcare team.
- Administer medications and manage medical equipment, including IV pumps, drains, and tubes.
- Educate patients and families about care plans, helping them understand treatment options and supporting them through the process.
- Attend bi-weekly staff meetings and monthly reviews to collaborate on patient care strategies and team development.
- Participate in a daily 10-15 minute morning call with the Clinical Team Manager to discuss overnight developments.
**Requirements:**
- Current RN licensure in South Carolina (ADN or BSN accepted).
- BLS certification preferred.
- Experience in hospice, oncology nursing, or home health care is highly desirable.
- Friendly, outgoing personality with a passion for working with hospice patients and their families.
- Must have healthy boundaries and be skilled in de-escalating situations.
- Ability to travel within a 50-mile radius for patient visits; reliable transportation is required.
**Work Schedule:**
- Monday to Friday, 8:30 AM to 5:00 PM (flexibility to start 30 minutes early and end 30 minutes early).
- Occasional on-call weekends may be required, typically when other staff members are on vacation.
- Most holidays are off.
