Position Summary: We are seeking a dedicated and highly motivated Floating Home Care Case Manager to join our healthcare team. As a Floating Home Care Case Manager, you will be responsible for overseeing 2 to 3 territories within our 21-county service area. Your primary role will involve providing comprehensive case management services to clients in their homes, ensuring they receive high-quality care and support. This position requires extensive travel within a 50-mile radius from your home, making it essential for you to be a road warrior. Responsibilities:
- Perform comprehensive assessments of clients' health, functional abilities, and support needs.
- Develop personalized care plans in collaboration with clients, their families, and healthcare providers.
- Coordinate and arrange home care services, including skilled nursing, therapy, and personal care assistance.
- Conduct regular visits to clients' homes to monitor their progress, assess the quality of care, and address any issues or concerns.
- Provide ongoing education and support to clients and their families regarding health management, medication adherence, and lifestyle modifications.
- Collaborate with a multidisciplinary team, including physicians, nurses, therapists, and social workers, to ensure continuity of care.
- Evaluate and revise care plans as necessary based on changes in clients' conditions or goals.
- Advocate for clients' rights and ensure their preferences and choices are respected.
- Maintain accurate and up-to-date documentation of all client interactions, assessments, and care plans.
- Stay informed about community resources, support services, and funding options available to clients.
- Participate in regular team meetings, case conferences, and professional development activities.
- Adhere to ethical and legal standards of practice and maintain client confidentiality.
Qualifications:
- Valid nursing license (RN) in the state of Minnesota.
- Proven experience in case management, preferably in home care or community health settings.
- Excellent assessment and critical thinking skills to develop comprehensive care plans.
- Strong communication and interpersonal skills to establish rapport with clients, families, and healthcare providers.
- Ability to prioritize and manage a caseload effectively, while maintaining attention to detail.
- Familiarity with local healthcare resources, community programs, and funding options.
- Proficiency in using electronic health record systems and other relevant software.
- Reliable transportation and a valid driver's license with a clean driving record.
- Flexibility to travel within a 50-mile radius from home and work independently.

- Ability to adapt to changing environments and work collaboratively in a team setting.
- Willingness to participate in ongoing professional development and continuing education.
