Our client is seeking a dedicated RN Case Manager with a passion for home health care to serve vulnerable demographics, including children at risk, seniors, chronically ill individuals, and those approaching end-of-life scenarios.

Essential Qualifications:

- A minimum of an associate degree.

- At least 1 year of professional experience in Medical-Surgical and Home Health Care.

- A current Registered Nurse (RN) license valid in Ohio (OH).

- Valid American Heart Association Basic Life Support (AHA BLS) certification.

- Minimum 1-year experience in a Burn Unit/Wound Clinic or Wound Care.

Preferred Qualifications:

- Bilingual proficiency in Spanish.

- Any specialized nursing certifications.

Brief Summary: 

Our client is currently seeking a dedicated full-time RN Case Manager in West Cuyahoga County, OH. The selected candidate will provide superior nursing care, possess robust communication skills, and maintain patient safety. The role calls for impeccable organizational abilities, the capacity to operate independently, and an unwavering commitment to maintaining confidentiality.

Coverage Areas: The position spans the regions within Summit County (Akron).

Responsibilities:

- Deliver direct or indirect comprehensive nursing care to a variety of age and diagnostic groups.

- Implement nursing process principles to facilitate data collection, planning, and evaluation of nursing care.

- Show essential nursing knowledge and skills in implementing prevention, therapeutic, and rehabilitative nursing care.

- Employ effective communication techniques with everyone from healthcare providers to patients, ensuring comfort and safety needs are met.

- Respond to critical situations according to nursing standards, policies, procedures, and protocols.

- Use EMR: Home Care Home Base and other provided equipment such as a tablet and phone for efficient task handling.

Shift Details:

Working hours are 8:30a-4:30p with expected availability for 1 weekend per month, including one summer and one winter holiday.
