We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager with a specialty in Hospice care to join our dynamic healthcare team.
The ideal candidate will be responsible for the coordination of care for a set group of patients to ensure the highest quality of holistic care is provided. This is a full-time permanent position that requires a unique blend of clinical expertise, patient advocacy, and emotional support. This role is an opportunity to make a significant and meaningful impact in the lives of patients and their families during their most challenging times.
Responsibilities
1. Provide comprehensive nursing care to patients
2. Coordinate the development and implementation of individualized patient care plans in collaboration with the interdisciplinary team.
3. Regularly assess the patient's condition, monitor vital signs, and adjust care plans as necessary.
4. Provide emotional support and counseling to patients and their families, helping them to navigate end-of-life decisions and challenges.
5. Liaise with physicians, therapists, and other healthcare professionals to ensure the seamless delivery of care.
6. Educate patients and their families on disease management, self-care techniques, end-of-life care, and the bereavement process.
7. Document patient information, care provided, and any changes in patient condition in a timely and accurate manner.
8. Ensure compliance with all federal and state hospice regulatory requirements.
9. Participate in on-call rotation to provide 24/7 patient support.
Qualifications
1. Current and unencumbered Registered Nurse (RN) license to practice in California
2. Ideally a minimum of 2 years of nursing experience, with at least one year in hospice or palliative care.
3. Experience in case management and development of individualized care plans.
4. Excellent clinical nursing skills and a deep understanding of the hospice philosophy.
5. Exceptional communication and interpersonal skills to provide comfort and support to patients and their families.
6. Ability to work independently, manage time effectively, and prioritize tasks.
7. Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
8. Current CPR certification.
