What You’ll Do:

• Provide skilled nursing care to hospice patients within our center-based facility.
• Conduct thorough assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Administer medications, monitor vital signs, and manage symptoms to ensure patient comfort.
• Educate and support patients and their families regarding end-of-life care and symptom management.
• Coordinate care with other healthcare professionals within the center.
• Offer emotional support, counseling, and education, addressing the unique needs and concerns of patients and their families.

About You:

• Active RN license in the state of practice, or eligibility to take NCLEX for new grads.
• Minimum of 1 year of experience in hospice, palliative care, or a related field preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to collaborate effectively within a multidisciplinary team.
• Compassionate and empathetic approach to patient care.
• Knowledge of hospice regulations and practices preferred.
