Responsibilities:

Coordinate and manage patient care plans

Conduct regular assessments and update care plans

Collaborate with interdisciplinary teams

Educate patients and families on care processes

Ensure compliance with hospice regulations

Qualifications:

Valid RN license in California

Minimum 1 year of hospice or related experience

Strong communication and organizational skills

Ability to work independently and in a team
