Job Description:
POSITION SUMMARY: The Hospice Nurse admits patients to service, assesses the patient s and family s physical, psychosocial, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice programs' quality assessment performance improvement program and conducts follow up visits as needed.
Territory Details: Campbell, Los Gatos, Saratoga, San Jose, Milpitas, Gilroy, Coyote Point, Morgan Hill, St. Martin, Cupertino, Sunnyvale, Santa Clara, Mountain View and Neighboring cities as needed.
Requirements: One year of Hospice experience.

Shifts:
Days: 8:30am-5pm (Schedule can be flexible based on patient's availability.) Weekends: One weekend per month.
