A-Line Staffing is now hiring a Care Manager located in Columbia County Florida. The Care Manager would be working for a major company and has career growth potential. This would be full time / 40+ hours per week.

Care Manager Compensation

• The pay for this position is between $40 and $45 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Care Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday to Friday 8am to 5pm
• This role will require some local travel in Columbia County Florida including home visits within 60 miles of your location

Care Manager Responsibilities

· Would be doing case management for children medical services (0-21)

- home visits upon member enrollment and performing monthly or quarterly outreach

·Attending meetings, meetings for members, participation in regional meetings and activities that are required by the team.

·All documentation is completed through TruCare, Teams, Zoom.

· In addition, care coordination which includes but is not limited to educating members on resources, travel, assisting members with locating physicians, therapies, providers, DME, etc., completion of assigned trainings, submitting authorizations

Care Manager Requirements

• Registered Nurse with Active State License (FL)
• Pediatric Case Management experience
• Associates, Bachelor’s, or higher degree or accredited nursing diploma
• 2+ years of clinical nursing experience with at least 1 year in Managed Care
• Strong Computer skills, including e-mail, word processing, spreadsheet and web-based programs

Care Manager Preferred Qualifications

• Trucare and Medicaid experience
• 5+ years of pediatric case management experience
