About The Position
**Job Title:** Care Manager **Job Specialty:** ED Utilization Management **Job Duration:** 9 Months **Shift:** Day - 5x7.5 - 9am - 5pm, Monday to Friday **Guaranteed Hours:** 37.5 Hours per Week **Experience:** Minimum of 5 years of clinical experience as a Registered Nurse **License:** Registered Nurse, Licensed Practical Nurse, or Physical Therapist **Certifications:** 1. RN Licensed as a Registered Nurse in the State of New York required. 2. Baccalaureate degree required. 3. PRI and Screen certification within three months of employment is required. 4. Master's Degree and bilingual Spanish Certification in Case Management preferred. **Must-Have:** - UM/UR experience in managed care or hospital setting required. - Leadership experience preferred. - Experience with care coordination, utilization review, and discharge planning. - Computer literacy required. **Job Description:** - Perform utilization management and review tasks. - Engage in effective care coordination and discharge planning. - Utilize clinical expertise to assess and determine appropriate level of care. - Collaborate with a multidisciplinary team to enhance care delivery. - Ensure compliance with state and federal regulations regarding patient care. - Maintain detailed and accurate records of patient interactions and care plans.
