ID: 51887512 Shift: Day 5x8-Hour (08:00 - 16:30) Description: Must have acute hospital experience in CM and discharge planning- Do not submit CM's with only SNF, REHAB or Hospice experience- Must have prior travel experience- Certification is not required.- Must be willing to work in weekend rotationREQUIRED-1 yr. minimum recent experience with discharge planning as a Clinical Case Manager with a heavy focus on discharge planning in an acute care hospital setting- Need to have acute INPATIENT case manager experience.- BLS- InterQual- EPIC- Allscripts/CareportPREFERRED- 2 or more years- Utilization Review is a plus- ACM or CCM Certification is a plus Modified 12:00:00 AM Account Manager: Sara Mitchel Account Manager Email: COVID-19 Vaccine: Required - Medical/Religious Exemptions Only Flu Vaccine: Unknown Submittal Details: #Tier3 Travel ComplianceCompliance document due date is the Wednesday prior to start.Start date Due to compliance, UC Davis will book for a start date approximately 3 weeks out. If a candidate gets compliant early, facility will consider moving them to an earlier start.Compliance Booked candidates must be actively committed to becoming compliant, including signing two documents within 48 hours of accepting contract.
