Essential Functions:





+ Conduct Home Health Aide skills trainings, competency evaluations, HHA introduction and 90 day evaluations


+ Complete initial client assessment, plan of care, 90 day reassessments, and ongoing evaluations of the GAFC setting


+ Partake in both care plan creations and reviews, along with discharge plans


+ Assess and address motivational, psychosocial, and environmental concerns


+ Maintain and update client charts, medical records and submit all relevant information to authorized organizations


+ Completing notes corresponding with each on-site visit or encounter, or more often as the member's condition warrants


+ Document health changes and communicate to the client's Case Manager and Program Manager in a timely manner


+ Review all paperwork and plans of care to ensure program compliance, updating information as needed


+ Performs any other additional duties that may be required by management at any time




Essential Requirements:




+ Travel is required


+ Valid drivers license and reliable transportation required


+ MA RN license required


+ Minimum of 1 year experience in an acute or home health care setting or comparable work experience preferred


+ Fluency in both English & second language preferred


+ Case management, social services, and/or community health worker experience preferred


+ Past experience working with behavioral health population is a plus


+ Strong communication, writing, organizational, and time management skills


+ Solution oriented with the ability to multitask
