Our client is part of a group of long-term care affiliates at Riverdale - North Bronx is looking for a RN - CHHA-Intake/Outreach Manage
Type: Full-Time/ Perm
Schedule: Hours: 9am-5pm Days
Location: Bronx, NY
Responsibilities:
The Intake/Outreach Manager will be responsible for managing the screening, evaluation, and admission planning for the CHHA Program.
This position will manage the process from inquiry to admission acceptance, coordinate and assist with the scheduling of the Nurse Care Manager, internal and external vendors to ensure timely evaluation and admission.
In collaboration with the Associate Administrator of Provider Relations, establishes new referral sources.
May perform the direct services of a Nursing Care Manager.
Qualifications:
Must possess a current NYS RN License and registration.
Bachelor of Science degree in Nursing required with a minimum Two (2) years of experience assessing patients for home care safety and developing appropriate care plans and plan adjustments and a minimum of One (1) year supervisory experience in hospital or home care agency.
Patient Review Instrument (PRI) assessor skills experience preferred.
Valid driver's license required.
MS Office required.
Bilingual preferred.
