Reporting to the Resident Assessment Manager, the Clinical Assessor is responsible for the implementation of Resident Assessments, using the MDS 2.0 and PRI. The Clinical Assessor will also conduct periodic reassessments of Medicare-eligible residents, participate in interdisciplinary team meetings, and serve as a resource for the staff regarding appropriate medical record documentation for MDS.

Position Qualifications:

This position requires a current New York State Nursing license and three years of experience in a long term care facility as an MDS Coordinator, along with knowledge of Medicare criteria and assessment regulations. The ideal candidate must possess excellent listening and communication skills, the ability to complete documentation in a timely manner, as well as attention to detail and accuracy when performing assessments.
