As the Registered Nurse, you will be responsible for providing and coordinating multidisciplinary home health services for patients in the community. You will work with the physician and other team members in the implementation of the plan of care.

Responsibilities include:
- Provides and coordinates patient care that is cost-efficient and reimbursable under the patient's insurance plan and approved by physician
- Accepts case assignment depending on client's needs in specific geographic location(s)
- Case assignment is all-inclusive of CHHA requirements, including initial assessments, case opening, and documentation on first day of service
- Travels to patients' homes and/or other facilities using approved transportation to deliver direct care to the patient
- Ensures in-person completion of the Oasis that accurately reflects clinical and functional status of the patient. Uses diagnosis Coding skills to accurately rate the acuity of each diagnosis in the Oasis.
- Obtains and evaluates HHRG and HIPPS score and works with the Supervisor on matching the scores to frequencies and duration of services on 485 (physician orders)
- Evaluates patient diagnosis and work with RN supervisor and physician to assign an appropriate primary diagnosis.
- Participates in weekly PPS meetings to report adherence to frequency and duration of services as per physician's order. Reports patient progress to PPS team so that discharge date can be set and anticipated.
- Communicates frequently with PT/OT to ensure adherence to the frequency and duration ordered by the physician.
- Acts as a coordinator of the multidisciplinary team providing care to patients and establishes a care plan with input from appropriate health team members.
- Documents content and process of all services provided and monitors the effect of care plans on the patient's health status.
- Educates the patient and family regarding the disease process, self-care techniques, and prevention strategies.
- Plans, facilitates, and documents patients' discharge from the program
- Admits patient to the program as needed and completes required documentation in a timely manner.
The ideal RN candidate will:
- Have at least a minimum of one year of experience as a Registered Nurse from an accredited institution.
- Prior nursing experience in long-term care or CHHA is preferred but not required-will train new graduates!
- Must be able to make home visits in potentially unsafe and unsanitary home environments (e.g., elevated buildings, walk ups, care facilities, single/multiple family homes, presence of pets, etc.).
- Access to reliable transportation or able to access public transportation reliably and able to transportation within Brooklyn, Queens and Manhattan, depending on case assignment. Standing, sitting, walking, bending, reaching, stretching, and climbing stairs.
- Able to lift up to 50 pounds unassisted and up to 325 pounds utilizing an assistive device.
