At Enhabit, our Registered Nurse (RN) Case Manager is the point of contact for all disciplines involved with providing care to patients.

• Performs skilled nursing visits 
• completes the coordination of patient care
• oversees the frequency of visits 
• Consults with the physician when as needed, and the branch director about patient care
• Accurate OASIS collection and documentation
• ensures the medication profile remains current
• timely reporting of lab values to physician
• attend weekly case conference
• monthly case manager meeting

Schedule: Monday - Friday 8am - 5pm

On-Call Rotation

• Registered Nurses (RN) licensed in the state
• 1 Year of Home Health RN experience required
• Oasis experience preferred
• Demonstrate knowledge and skill in current nursing practice
• Possess a valid state driver’s license and automobile liability insurance
