Lead with Heart - Nurturing Care Coordination
Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides, as we cultivate a harmonious team of caregivers. In your role as a facilitator of care coordination, we celebrate the unity that arises when PRN staff steps in, ensuring seamless support for your patients.

Qualifications
+ Active RN license in state of service
+ One (1+) year of clinical experience required; One (1+) year of hospital or home health experience preferred
+ Current CPR certification
+ Valid Driver’s License
