This role involves providing Home Health care to patients in the comfort of their own homes, ensuring they receive the highest standard of care. Top Skills Required:

• 1+ year of professional experience as an RN (Home Health experience preferred)
• Active RN License in the State of PA
• Associate Degree (or higher) in Nursing
• American Heart Association Basic Life Support (AHA BLS) Certification
• American Heart Association Advanced Cardiac Life Support (AHA ACLS) Certification
• Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance

Responsibilities:

• Delivering quality Home Health care to patients in the local area.
• Adhering to established health and safety guidelines and policies.
• Documenting patient care services by charting in patient and department records.
• Maintaining continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
• Participating in the comprehensive 6-week RN onboarding program.
• Providing exceptional care by assessing patient health and creating care plans.
• Coordinating with physicians and other healthcare professionals to ensure cohesive patient care.
• Educating patients and their families on health care plans, medications, and self-care techniques.

What You Need to Know:

• Schedule: Monday - Friday, 8am-5pm with on-call weekend rotation every 4-8 weeks depending on staffing levels.
• Flexible schedules with minimal weekend shifts per year.
• Travel Position with travel reimbursement provided.

Candidate Details:

Candidates must have a valid RN License in PA, an Associate Degree or higher, and BLS/ACLS certifications. They should have a valid driver's license, a reliable mode of transportation, and auto insurance. Experience in Home Health, Med/Surg, Telemetry, any critical care, Emergency Department, or long-term care is preferred, but new nurses are also welcome. Candidates must be comfortable with the required travel for the position and should not have applied to Residential Healthcare Group in the last 6 months.
