About The Position
**Job Title:** Registered Nurse (RN) I **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** Not guaranteed **Experience:** Minimum 1 full year Adult Home Health or Hospice experience required **License:** Must have license in hand **Certifications:** CPR, BLS **Must-Have:** Proficiency in OASIS and HomeCare HomeBase (HCHB) EMR **Job Description:** - Provide, coordinate, and direct home nursing care based on physicians' orders and agency policies. - Use the nursing process competently for patient care. - Supervise and evaluate the work of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Travel to home visits and attend periodic office meetings. - Have reliable transportation, a current driver's license, and agency-required liability insurance. - Handle an average of 5-8 patients per day, meeting specific productivity expectations. - Manage types of cases such as Start of Care, Wound Care, and Follow-up Visits. - Perform assigned tasks covering a territory radius of up to 75 miles from the home clinic. - Mileage reimbursed from patient home to patient home per IRS standard rate; the commute to and from the first and last patient is not billable. - Be prepared for variable weekend and on-call requirements, differing per location and profession. - Familiarity with charting systems and EMR, specifically requiring recent experience with HomeCare HomeBase within the last six months. - Open to on-call rotations and traveling to locations within the assigned territory. - Meet the weekly productivity expectations of 30 points with a clear understanding of how points are broken down across various tasks.
