About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day, 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** No **Experience:** Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience **License:** Must have license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in HomeCare HomeBase (HCHB) and OASIS - Reliable transportation and a current driver's license - Ability to travel up to 75 miles from the home clinic - Open to on-call rotations every 3-4 weeks - Willingness to relocate to the assigned territory - Comfortable with completing all documentation within 24 hours of patient appointments **Job Description:** - Provide, coordinate, and direct home nursing care according to physician orders and agency policies. - Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Manage a patient caseload with an average of 5-8 patients per day. - Perform various types of patient care, including Start of Care, Wound Care, and Follow-up Visits. - Meet weekly productivity expectations of 30 points. - Ensure timely documentation and adherence to compliance standards. - Engage in weekend rotations and be flexible with schedule changes based on staffing needs. - Travel between patient homes and the clinic for weekly meetings, with mileage reimbursed at the standard rate. This position requires strong clinical skills, the ability to manage multiple patient cases, and excellent communication abilities to ensure high-quality patient care and effective team collaboration.
