About The Position
**Job Title:** Registered Nurse I **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day Shift, Monday to Friday, 8:00 AM to 5:00 PM **Guaranteed Hours:** Not guaranteed **Experience:** Minimum of 1 full year of Adult Home Health or Hospice experience **License:** Must have a current and valid nursing license **Certifications:** CPR and BLS required **Must-Have:** - Proficiency in OASIS and experience with HomeCare HomeBase (HCHB) EMR within the last year - Reliable transportation, a current driver's license, and liability insurance - Comfort with on-call duties and weekend rotations - Willingness to travel up to 75 miles from the assigned care center **Job Description:** - Provide, coordinate, and direct home nursing care according to physician orders and agency policies. - Supervise and evaluate Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses. - Perform home visits and periodic office meetings. - Manage a caseload with an average of 5-8 patients per day. - Complete documentation within 24 hours of patient appointments. - Meet weekly productivity expectations of 30 points, equating to approximately 40 hours per week. - Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits. - Travel between patient homes, covering approximately 150-200 miles per week, with mileage reimbursed at the IRS standard rate. - Engage in on-call rotations and weekend assignments as needed. - Accept every patient assigned, regardless of travel distance between appointments.
