About The Position
**Job Title:** Registered Nurse I **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** None **Experience:** Minimum of 1 full year of Adult Home Health Experience or 1 full year of Hospice experience **License:** Must have a valid nursing license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficient in HomeCare HomeBase (HCHB) and OASIS, with use within the last 6 months, or completion of a retraining course during compliance. - Vehicle, driver’s license, and auto insurance. **Job Description:** - Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures. - Evaluate and supervise Home Health Aides and Licensed Practical/Vocational Nurses. - Drive to patients' homes daily; participation in periodic office meetings is required. - Responsible for handling cases such as Start of Care, Wound Care, and Follow-up Visits. - Will travel up to a 75-mile radius; mileage reimbursed at the standard rate. - Ensure all documentation is completed within 24 hours of the appointment; delayed documentation may result in cancellation. - Participate in an on-call rotation, typically one weekend per month. - Meet weekly productivity points of 30, equating to roughly 40 hours per week. - Must be open to patient assignments, regardless of distance from the previous appointment. - Open to floating to another nearby clinic to pick up patients, if necessary. Please note that locals are accepted at the same rate. All travelers must be open to on-call rotations and able to travel up to 75 miles from the care center.
