About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** Not specified **Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience **License:** Must have a valid RN license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in OASIS and HomeCare HomeBase EMR (used within the last 6 months) - Reliable transportation, driver's license, and liability insurance able to travel up to 75 miles from the home clinic - Willingness to undergo a retraining course if proficiency in OASIS and HomeCare HomeBase EMR was not utilized in the last 6 months **Job Description:** - Provide, coordinate, and direct home nursing care according to physician orders and agency policies through the application of the nursing process. - Responsible for the supervision and evaluation of Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses. - Perform home visits and participate in periodic office meetings. - Maintain an average of 5-8 patients per day, with productivity expectations of a minimum of 30 units weekly. - Conduct a variety of cases, including Start of Care, Wound Care, and Follow-up Visits. - Meet documentation requirements within 24 hours of patient appointments to ensure timely billing and follow-up care. - Engage in weekend rotations and on-call duties, as per staffing and location requirements. - Participate in weekly meetings at the clinic and manage a patient load with travel expected between patient homes. - Maintain flexibility in scheduling, including potential work in nearby clinics to meet patient census needs. - Ensure all patient appointments are accepted, regardless of location. - Participate in any necessary orientation or training sessions required by the agency. **Note:** Please confirm all job specifics and expectations during the interview process.
