About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** Not guaranteed **Experience:** Minimum of 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty **License:** Must have a valid RN license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in OASIS; must have used it within the last year - Proficiency in HomeCare HomeBase EMR; must have used it in the last year - Must be willing to travel up to 75 miles from home clinic - Reliable transportation and valid driver's license with required liability insurance - Open to on-call rotations and weekend assignments **Job Description:** - Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process - Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Conduct home visits and attend periodic office meetings - Responsible to the Clinical Manager - Manage start of care, wound care, and follow-up visits - Work with an average of 5-8 patients per day, meeting productivity expectations of approximately 30 points per week, with different types of visits and activities contributing to point values - Be prepared to discuss weekend and on-call requirements during the interview. - Must be proficient in charting and meet agency documentation standards Please note that locals are accepted at the same rate, and there is no acceptance of pending licenses for this position.
