Brief Summary
The RN Case Manager provides quality nursing services to individuals in their homes effectively utilizing the development, implementation, and evaluation of patient care plans. The RN Case Manager acts as a liason between the Home Health Agency, the Patient, and the patient's Family. The RN Case Manager carefully follows professional standards of care, policies, and procedures.

Professional Breakdown of a Home Health Registered Nurse
- Develops a plan of care that corresponds with a physician's orders through an assessment of the patient's needs, their condition, and their environment.
- Collaborates with other Health Team Members during the advent of the initial visit and updates the plan of care as changing circumstances require. Requires excellent communication skills.
- Supervises LPNs and paraprofessionals in the administration of care. Maintains an ongoing responsibility for assigned caseload.
- Demonstrates a genuine compassion for patients and strictly upholds appropriate staff-patient boundaries.
- Maintains communications regarding Patient needs with the attending Physician, the Director of Nursing, the CNA Supervisor, and other staff as needed.
- Provides health education and disease prevention to patient and family on any diagnosis, medications, and/or prescribe treatments as ordered by the Physician.
- Provides teaching and counsels patient and family members.
- Provides skilled nursing care that conforms with the highest standards of patient care and generally accepted medical practices and policies.
- Rotates weekends, nights, and holidays with other staff members in order insure proper coverage for all patients.
- Completes clinical and progress notes strictly within required timeframes. OASIS experience is preferred. Qualified candidates unfamiliar with OASIS will be trained.
- Attends interdisciplinary treatment team meetings designed to facilitate proactive patient care.
- Plans for a patient's discharge from service.
Job Requirements
- Education- a minimum of an Associate Degree in Nursing (ADN), followed by successful completion of the NCLEX-RN exam.
- Professional License- a current Utah Registered Nurse License. The current status must be maintained.
- Experience- is a relative term. An aptitude for learning and an unabashed work ethic are highly valued. Two years of experience is helpful and is weighed along with one's learning ability and demonstrated work ethic.
- Must pass a background check required by the state of Utah.
- Must have reliable transportation, a valid driver's license, and proof of insurance.
- CPR training is required.
