Now Hiring: RN/LPN MDS Coordinator

RN/LPN MDS Coordinator Responsibilities: 

• Conduct comprehensive resident assessments to develop and update individualized care plans.
• Ensure accurate and timely completion of the Minimum Data Set (MDS) for each resident.
• Collaborate with interdisciplinary teams to gather and analyze information for resident care planning.
• Maintain compliance with state and federal regulations related to MDS assessments.
• Provide education and training to nursing staff on MDS process and documentation requirements.

RN/LPN MDS Coordinator Qualifications:

• Licensed RN or LPN Registered Nurse in the state of Florida
• Previous LTC/SNF experience preferred
• All applicants must be able to pass a Level 2 background check
• Prior experience with Resident Assessments and MDS in Long-Term Care Settings required
• RAC-CT certification preferred & able to obtain after hire
