Acacia Network is one of the largest providers of transitional housing in New York City, providing an array of resources to individuals and families in more than 50 shelters across the City, including one-on-one case management with social workers, access to primary and behavioral care, employment assistance, among other services.
Acacia Network Housing (ANH) -the Acacia division dedicated to homeless services- serves over 7,000 individuals across our transitional housing portfolio, including 2,500 families with children and 2,000 single adults. This is equivalent to roughly 13% -or 1 in 8- of all individuals served through the New York City shelter system. The Welcome Centers are designated as a stabilization site, a place where street homeless New Yorkers can find accommodations without going through the sprawling shelter system intake process.
Our integrated care model ensures that individuals and families served through our shelters are able to benefit from our full continuum of services, including Outpatient Integrated Services, Primary Care, Mental Health Services, Addiction and Substance Use Treatment, Residential Care, Arts & Culture Workshops, among other resources. Our goal is to help individuals and families regain stability so they can secure permanent, affordable housing, and achieve personal quality-of-life goals.
The Registered Professional Nurse Manager is responsible for coordinating and delivering patient care utilizing the nursing process in the Acacia Network Housing Welcome Centers (Brooklyn). The Registered Nurse participates as a member of the interdisciplinary team providing support and information about nursing services. The RN Coordinates care between other professional disciplines as needed. Through documentation and verbal communication, essential information is shared with health care team members. When care is not provided directly by the Registered Nurse, the RN assigns/delegates care to other qualified members of the health care team.
The salary is $110,000/Annually
- Supports and promotes the philosophy, objective, policies and procedures of Acacia's Care Facility and the Nursing Department

- Keeps the Program Administrator on call, informed of facility problems that need concern
|
- Responsible for providing leadership and supervision to the nursing staff and related level staff.

- Will greet patients and prepare them for physical examination and treatment.
|
- Will maintain medical supplies and equipment.
|
- Provides education as indicated after assessing the learning needs of the patient, staff members and students.
|
- Assess the patient's physical and psycho-social condition on an ongoing basis throughout the continuum of care.
|
- Utilize manual and automated information systems to improve efficiency and effect productivity.
|
- Demonstrate a current working knowledge of equipment, instrumentation and medications.
|
- Adhere to the standards, policies, procedures and goals of the medical center.
|
- Act as a liaison in coordinating patient care activities with nurses, physicians and allied health professionals.

REQUIREMENTS:
BS in Nursing
3 years experience
Experience working with underserved populations preferred
Licensed and currently registered as a Professional Nurse in New York State
Bilingual Spanish Preferred
CPR Certification
