The RN Medical Care Manager contributes to AHF by utilizing strong nursing clinical skills and understanding of the Ryan White Program within their specific area. With a caring and knowledgeable focus, you will work with our physician and other providers, the Care Management team, and the Healthcare Center team so that every Ryan White patient receives the support they require and access to available resources. This will include providing education and follow-up with the clients. Working directly within the Healthcare Center, you will meet with patients and complete Needs Assessments, develop Care Plans, and provide ongoing education to clients so they understand their diagnoses and their medications. Working with your team allows for full collaboration to meet all aspects of your patient’s health and psychosocial needs. In this way, you will support the patient’s ability to understand their health status better to work to achieve their healthcare goals.
The RN Medical Care Manager enhances the reputation of AHF by utilizing core principles of nursing, you will meet our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your collaboration and engagement skills, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. By establishing Needs Assessments, Care Plans, referrals, and support, your work will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The RN Medical Care Manager will work as a part of a close-knit team to provide coordinated healthcare interventions to assist patients to self-manage their HIV. It will also provide self-management, including making and keeping healthcare appointments, taking ARTs and other prescribed medications as prescribed, and communicating with healthcare providers to coordinate care for comorbid chronic or disease conditions.
Licenses and Certifications: Bachelor’s degree in Nursing (BSN), or License Practical Nurse (LPN) with a minimum of 1 year of Case Management experience.
Knowledge of Ryan White Services is preferable.
