General Certification Requirements (BLS, etc.) BLS,
CCM or ACM preferred not required
Case load/# patients per day/consults per day can be up to 15 patients

Is this position for UR, discharge planning, or both? only discharge planning
What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.) inpatient only
Common types of patients & job duties Will see primarily adults on all floors and ER
Specific job duties: Discharge planning- will review the patients and work towards a discharge plan, with high risk patients will do an in-depth interview and work towards a discharge plan. Will work closely with interdisciplinary team including all providers and therapies
Experience or 'Must have' skills - Must have minimum of 2 years experience in an acute inpatient setting, will consider a first time traveler if they have a good background in acute care case management
Is there a CM Manager/Supervisor on each shift? manager or supervisor
Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) 8 hour shifts, 0730-1600 with 30 minute break
May be asked to do some 10 hour shifts- manager would discuss with traveler onsite if this was needed. If covering ED would work 12 hour shifts, 0800-2000 or 10 hour mid shift starting at noon
Weekend Requirements as needed 8 hour shifts 0730-1600
Holiday Requirements if there is a need
Scrub Color/Dress Code business casual
RT for DC Planning? yes
Pharmacy for DC Planning? yes
How many shifts of unit/department specific orientation? 6 days in department
Additional Notes Culture is very important to the department, they are in a city but not a super big place- has a community feel and are very supportive in the work that we do. Would like someone that can delegate as needed but is also on top of their work and has good communication- will be doing a lot of back and forth collaboration. Will be working with case management assistants that assist with phone calls, faxing, placing referrals once they have the patient choices on where they would like to go.
