+ Department: Primary Care Clinics
+ Schedule: Monday - Friday, 40 hours/week, No Nights, No Weekends and No Holidays
+ Hospital: Joshua Max Simon Primary Care Center at Ascension Saint Vincent
+ Location: Indianapolis, Indiana
_This role is responsible for Quality Assurance best practices with implementation and education as well as the Medicare Wellness Visits with patients._

Responsibilities
Serve as a patient navigator throughout the continuum of care.
+ Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
+ Coordinate efforts for treatment and promote quality improvement and educational efforts.
+ Compile data, track outcomes, and make recommendations for process improvement.
+ Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
Requirements
Licensure / Certification / Registration:
+ Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
+ BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
+ Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
Additional Preferences
Additional preferences preferred are:
+ RN experience of at least 2 years and additional recent experience with Quality Assurance best practices.
+ Recent experience as a RN in the Ambulatory Setting performing Quality Assurance duties and experience with influencing and implementing qualtiy improvements and education.
+ Experiene with performing the annual Medicare Wellness visits as a RN..
+ Great assessment skills.
+ Excellent Customer service and teamwork.
+ Bilingual skills preferred.
