Job description/Responsibilities: Full Time - (40 hours per week) AM Shift APPROXIMATE PAY RANGE: $55.26 - $64.13 JOB SUMMARY: The Registered Nurse provides direct care in the Skilled- subacute unit. Communicates with physicians / Nurse Director / Co-workers, as appropriate about changes in the patient's clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. DUTIES AND RESPONSIBILITIES 1. Consistently uses two patient identifiers when drawing lab, administering medications, and performing procedures. 2. Does not use unacceptable abbreviations in documentation, (using read back on all telephone orders.) 3. Consistently follows medication reconciliation policy. 4. Ensures Crash Cart, defibrillation function has been checked per the protocol and after each code, appropriate items are immediately replaced and indicated by signature on crash cart checklist. 5. Is in compliance with the information contained in the Infection Manual (i.e. OSHA bloodborne pathogen standards, CDC Guidelines ), and demonstrates appropriate use of personal protective equipment (i.e. gloves, gowns, masks, goggles ) and handwashing techniques. 6. Able to give a thorough, detailed efficient change of shift report to ensure consistency in the plan of care. 7. Assists physicians with special tests and procedures in the unit. Uses time out on all patients for any bedside procedures. 8. Avoids and prevents injury to self and patients by applying the principles of body mechanics. Maintains a safe environment, functioning with an awareness and application of safety issues identified within the unit/facility, i.e. suction and oxygen immediately available when needed. 9. Consistent with pain assessment and reassessment with the time frame after giving pain medications and upon discharge or transfer of patients. 10. Completes documentation of medications given to the patient on the MAR and narcotic sheet / Pyxis. 11. Recognizes signs of abuse and reports appropriately to the Adult Protective Services. 12. Initiates discharge and patient/family education planning with proper documentation on the patient record in Medi-Tech. 13. Establishes priorities of patient care based on essential needs and available resources of time, equipment, personnel, and supplies. 14. Formulates, maintains and updates a goal-directed plan of care on patients which is prioritized and based on patient outcomes. 15. Performs other duties as assigned or required.

Required Qualification: EDUCATION, EXPERIENCE, TRAINING 1. Must have a current and valid license as a Registered Nurse issued by the State of California. 2. BCLS upon hire and maintain current. 3.
