We are seeking a Registered Nurse (RN) Transitional Care Case Manager to:
- Help assure care coordination across the care continuum.
- Work with a dynamic, innovative and fun interdisciplinary team.
- Practice at the top of your license.
- Provide one on one attention to patients and their care.

Education:
Minimum – Bachelor's degree in Nursing (BSN).
Preferred – Master’s degree in Nursing or Health Care related field.
Work Experience:
Minimum – Three (3) years of relevant clinical nursing experience. Equivalent combination of education and experience will be considered.
Preferred –Surgical, trauma, and intensive care experience.
Licenses and Certifications:
Minimum – RN licensed in the state of Wisconsin CPR certification.
Preferred – ACMA certification as a case manager.
