We are seeking a Per DiemRegistered Nurse (RN) Transitional Care Case Manager to:
- Bridge inpatient to ambulatory care for medical, surgical, and trauma populations.
- Coordinate with interdisciplinary teams to ensure remarkable healthcare.
- Use clinical expertise to empower patients during post-acute hospitalization recovery.
- Be an integral part of a dynamic, innovative, and fun team.

Education :
Minimum - Bachelor's degree in Nursing.
Preferred - Master's degree in Nursing or Health Care related field.
Work Experience :
Minimum - Three (3) years relevant clinical nursing experience. Equivalent combination of education and experience will be considered.
Preferred - Surgical, trauma, and intensive care experience.
Licenses and Certifications :
Minimum - RN licensed in the state of Wisconsin. CPR certification.
Preferred - ACMA certification as a case manager.
