Under the direct supervision of the Wound Care Nursing Administrator and Program Director, provides patient
with care that is continuously evaluated, planned for, and revised as needed; patient’s status is
documented within the reassessments; patient care needs; interventions to meet the patient’s needs;
nursing care provided; the patient’s response to; and the outcome of care provided; organizes and
prioritizes work to provide health information, discharge planning, and patient care in a safe environment
to maximize health in the continuum of care and protect the patient; assumes responsibility for and
assigned group of patients and be accountable for an assessment of the patient’s basic human needs,
enabling team of providers to make appropriate decisions to develop, implement and evaluate a plan
for care, to include biophysical needs, bio-psychological needs, environmental/safety needs, self care
needs, educational needs, discharge planning needs; professional and competent as a care provider, a
leader of personnel, and a collaborator with other professionals; demonstrates awareness of his/her
unique position in the Department and responsibilities as a professional nurse and R.N. Case Manager
through accurate, clear, concise and timely written and oral communications.
