GENERAL SUMMARY
Provides direct services for consumers, and completes associated case management documentation for the consumer and program. Works within a multidisciplinary team to provide an array of direct (face-to-face) and indirect clinical services to adults in a person-centered, trauma informed, individualized focused approach utilizing evidenced-based practices delivered to fidelity. These services are delivered in multiple types of settings including field-based services in the individual’s home or other identified locations as well as clinic-based services. Individuals served may have multi-system involvement such is Criminal Justice, Child and Adult Welfare, Veterans, Homelessness, etc.
Specific Program Descriptions
Adult Services – Outpatient – The position works in the Outpatient programs and among the various Adult facility locations. Services in this program include individual and group outpatient services for adults which includes Crisis Services, Medication Management, Psychosocial Rehabilitation, Case Management, Skills Training, Complex Services. Other programs include Substance Use Disorder and Criminal Justice.
Adult Crisis Services – The position works in the Adult Crisis Unit or the Josephine Recovery Center. Services in the unit include emergency and planned crisis care for adults ages 18 and over who have a behavioral health diagnosis or need. The Adult Crisis Unit program provides meals, medication monitoring, supervised recreation, skills training, community referrals, and 24-hour supervision in a secure environment. The Josephine Recovery Center is not a secure environment, but provides the same services as The Adult Crisis Unit. In addition to these sites CHCS operates a Mobile Crisis Outreach Team (MCOT), a resource designed to bring a crisis worker into the community to provide a face-to-face assessment and intervention, follow-up and relapse prevention services.
SMART Program -- Specialized Multidisciplinary Alternate Response Team, a collaborative aimed at creating a multidisciplinary mental health response team in partnership with Bexar County Sheriff’s Office (BCSO), Acadian Ambulance and The Southwest Texas Regional Advisory Council (STRAC). The team consists of licensed mental health professionals, peer support specialists, mental health deputy and a paramedic. As part of this team, the SMART Sr. Care Manager will use specified triage criteria developed by Bexar County Dispatch team to identify low level 9-1-1 mental health related calls in order to dispatch the SMART team to respond. The target population are individuals experiencing a mental health crisis that are otherwise non-violent and in need of access to treatment.
ESSENTIAL DUTIES & RESPONSIBILITIES

- Provides direct clinical services and assessments. Develops treatment plan for consumers.
- Provide direct individualized clinical services 60% of hours worked.
- Participate in centralized scheduling process
- Participate in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned
- Ensures clinical documentation is submitted in accordance with CHCS standards, and contract specific requirements.
- Coordinate coverage for planned absences in advance with supervisor.
- Performs other related duties as required.
Specialized Program Duties & Responsibilities
SMART Program
- To assist the SMART team in providing care coordination and follow-up to individuals that the team responds/engages with
- Ability to provide the following services

- Other clinical interventions as assigned
- Participate in coordination and communication with BCSO and Acadian team members on the progress of individuals that are receiving services and navigation during linkage efforts by the SMART Team
- Facilitate access to centralized scheduling for individuals as appropriate
- Identify CHCS established patients and provide linkage back to assigned CHCS care manager to ensure their behavioral healthcare needs are being met
- Assess the social determinants of health on each individual that may affect their ability to control their mental health problems.
- Serve as the primary role to provide aggressive referral, linkage, and transition to the next level of care
- Coordinate CHCS intake function to Adult Behavioral Health services
- Complete all CHCS and contract specific required documents in timely manner and to standard
- Participate in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned
- Comply with and participate in Quality Assurance and Quality Improvement processes
- Coordinate coverage for planned absences in advance with supervisor
- Adhere to established CHCS Employee Handbook and policies and procedures
- To support the linkage efforts, the employee will use own vehicle to meet the needs as determined by the SMART Team.
- Will document all clinical contacts in Anasazi
- Performs other related duties as required.
MINIMUM ENTRANCE QUALIFICATIONS
Education and Experience
- Graduate Degree in Social Work, Psychology, Counseling, or a related behavioral health field
- Complete the Qualified Mental Health Professional Community Services (QMHP-CS) within six (6) weeks of hire or transfer
Licenses or Certifications

Other Requirements
- Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business
- Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies

PREFERRED QUALIFICATIONS
- One (1) year experience providing similar services
- Prior experience with or knowledge of trauma-informed care
- Bilingual (English/Spanish)
