Service Care Manager (Hybrid Various Locations in TX) from A-Line Staffing Solutions

Overview:

We are seeking a dedicated LTSS Service Care Manager to work remotely, with three days of travel for home visits and two days for documentation. This role involves assessing members for long-term care services and acting as an advocate to ensure comprehensive support.

Key Responsibilities:

• Conduct home visits to assess member needs and eligibility for personal attendant services.
• Document assessments and submit H2060 forms for service approval or denial within three business days.
• Develop tailored care plans for complex, high-risk members.
• Educate members and families on healthcare options and coordinate community resources.
• Maintain accurate documentation and ensure compliance with clinical and regulatory guidelines.
• Provide feedback on quality improvement opportunities.

Qualifications:

• Bachelor's degree and 2 – 4 years of related experience or LVN and 2 – 4 years of related experience
• Experience in roles such as Service Coordinator, Case Manager, LVN, or Social Worker.
• Strong communication and interpersonal skills.
• Ability to manage time effectively between field and remote work.
• Previous experience working with aged, blind or disabled population (5+ years or more)
• Social Work, LVN, Medical background that would require at least a bachelor’s for social work and a license for LVN is preferred.
• Will review candidate with a strong medical background without the degree.
