What is a Service Coordinator?

·         Informing participants of and referring participants to available community services.

·         Participating in the completion of comprehensive assessments and plans of care;

·         Writing service coordination notes in the participant’s health record upon service commencement, when significant changes occur, and at least one time every 90days or more often as necessary to ensure the health record reflects the current clinical needs of the participant.

·         Providing, as appropriate, coordination of the medically related physical, mental, and behavioral, service needs of a participant at the time of enrollment, during treatment and care in the Program, or at the time of discharge.

·         Assisting with any personal, social, family, or adjustment problems the participant may experience at the Program.

·         Referring the participant or family to the appropriate community resources if the participant or the participant’s family requires specialized counseling; and

·         Coordinating services if a participant needs services from other community agencies, and no agency is acting as coordinator of services for that participant. 

·         Assessing, planning, implementing, coordinating, monitoring, and evaluating the services required to meet a participant’s needs.

Qualifications of the Service Coordinator

• Bachelor’s degree in human services or a related field
• Current and in good standing discipline specific state license (if applicable)
• One (1) year experience of relevant and recent experience working with adults in a professional capacity.
