Summary
Catholic Charities of Acadiana is seeking a compassionate and dedicated Shelter Case Manager to join our team. Key Responsibilities
Service Coordination:
Provide comprehensive service coordination for shelter guests.
- Assist in the development and implementation of service plans, including housing placement, medical care, and other necessary services.
- Help guests obtain and maintain mainstream benefits.
- Maintain detailed and accurate records for each guest, documenting all services provided in Service Point within 24 hours.
- Advocate for guests and provide referrals to outside agencies.
Facility Maintenance:
- Ensure cleanliness of shelter guest rooms and maintain a clean, orderly office area and facility.
- Provide transportation and/or bus vouchers as needed.
Support and Training:
- Provide life skills training and monitor medication for guests as required.
- Coordinate service plans with other social service providers and attend scheduled service coordination staffings.
- Offer crisis prevention and intervention as necessary.
Communication:
- Regularly communicate with your Supervisor and attend all assigned training.
- Exhibit courteous and polite behavior at all times.
Additional Duties:
- Perform any other duties as assigned by the Supervisor.
Job Qualifications
- Education: Minimum of a high school diploma/GED required; Bachelor's degree preferred.
- Experience
- At least two years of service coordination or related social service experience.
- Knowledge of homelessness and housing programs is helpful.
- Experience working with individuals experiencing homelessness.
- Exceptional problem resolution and de-escalation skills.
- Strong oral and written communication skills.
- Ability to maintain appropriate behavior and personal boundaries.

- Medium: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. 100% of the time.
