POSITION SUMMARY

Provides goal-oriented activities that assist seriously and persistently mentally ill adults by locating, coordinating, and monitoring necessary care and services.  The major objective of services provided is to monitor, clarify and assist clients in personal benefit administration.  Services vary per individual needs, and may include obtainment of benefits, payeeship and/or entitlements.

VITAL DUTIES AND RESPONSIBILITIES [Major Job Functions]

- Works in a team setting to manage a case load consisting of individuals receiving any of the residential services, outreach services, or monitoring services necessary to help them live within the community.

- Assists clients in personal financial administration.

- Complete SSI & SSDI applications using the SOAR model

- Communicates with local SSA  & DDS representatives

- Tracks application outcomes using OAT tracking

- Provides appropriate therapeutic interventions.

- Manages aspects of BPHC/AMHH applications, reassessments, and DMHA audits

- Assists clients as Authorized Representative for Medicaid benefits applications/maintenance of benefits.

- Completes and monitors transitional loans for CSS clients

- Completes system updates on client packages including prior authorizations.

- Completes electronic medical record documentation as required.

EDUCATION AND EXPERIENCE

- Bachelor’s degree in psychology, social work or related human services field is required.

- Previous experience working in mental health or related discipline preferred.

REQUIRED CERTIFICATION/LICENSE

-  Successful completion of OBHP (Other Behavioral Health Professional) educational requirements is required within (30) days of hire date.
