The Social Determinants Coordinator coordinates program activities and analyzes data to support the Social Determinants program Community-Health Manager and Workers (CHWs) who provide social support, navigation and advocacy to high-risk low-income patients within the communities they serve in order to help them reach their health goals.
This is a hybrid position with 2 days in the Edison, NJ office. May have to travel to different sites in the network. Experience with Google Looker, SPSS and Tableau is strongly preferred as is experience in public health, IT or health administration.
Responsibilities:
A day in the life of an Social Determinants Coordinator at Hackensack Meridian Health includes:
- Coordinate the retrieval of multiple program rosters. Correct formatting of rosters to enable proper Epic upload.
- Analyze health system data to support multiple programs and identify patients who are at high-risk for poor outcomes.
- Manage and coordinate assignment of all referrals to the programs from other departments and external entities.
- Analyze data collected by the CHWs and develop reports, displaying program metrics, of the programs for key stakeholders.
- Develop and test unit process workflows between the SDOH department and other clinical departments. Create graphics for finalized versions of workflows for distribution.
- Develop graphs, charts, and reports. Manage databases to visualize program expansion and outcomes.
- Train and onboard new CHW managers and CHW team members to Epic processes and templates..
- Regulate the creation and submission processes of policies for multiple CHW initiatives.
- Coordinate the communication process for targeted eligible patients. Manage responses of batch communication.
- Establish and oversee the use of the Mitigation Fund for patients for the programs.
- Coordinate with IT and Epic teams for new builds and updates.
- Coordinate and harmonize with external vendors for data reporting and analysis.
- Maintain and update project plan in support of project operations.
- Other duties as assigned.

Education, Knowledge, Skills and Abilities Required:
- Four year Bachelor's degree from an accredited college or university.
- Excellent organizational skills.
- Excellent written and verbal communication skills.
- Ability to carry our written and oral instructions.
- Ability to exercise judgement in the application of professional services.
- Experience with data analysis/collection/management.
Education, Knowledge, Skills and Abilities Preferred:
- Masters preparation a plus.
- Experience service in low-income communities.
- Proficiency in Google Workspace, data entry and data visualization.
