Social Service Coordinator - Bayshore Residence and Rehabilitation Center

Bayshore Residence and Rehabilitation is recruiting for a Social Service Coordinator candidate to become a part of our team!

Duties of Social Service coordinator:

• Maintaining and/or improving resident's ability to control everyday physical and mental/psycho-social needs.
• Oversee/Manage the social service team's daily tasks/job duties.
• Identifying individual social and emotional needs

• Developing and maintaining individualized care plans
• Participating in the planning of the resident's admission, community re-entry, or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident.
• Build connections with area healthcare partners to help with admissions and census growth.
• Coordinate the pre-admission process and act as the liaison between the customer and the Facility.
• Facilitate an admissions process that is in accordance with State and Federal Regulations
