What will I do as a Social Service Coordinator?
- Independently, develops, organizes, implements, evaluates, and directs the social service programs of this facility and assumes accountability of Social Service department.
- Initiates in the development, administering, and coordinating of department policies and procedures.
- Keeps abreast of current federal and state regulations, as well as professional standards, and makes recommendations and modifies policies and procedures as necessary and appropriate.
- Reviews department policies and procedures, at least annually, and participate in making recommended changes.
- Develops and implements policies and procedures for the identification of medically related social and emotional needs of the resident.
- Participates in discharge planning, development and implementation of social care plans and resident assessments.
- Performs administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required. (e.g. evaluation of staffing needs, budget preparation and compliance).
- Assists in arranging transportation for residents to other facilities or to home when necessary
- Refers resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident. (e.g. absentee ballots, community wheelchair transportation).
- Provides information and assistance to resident/families in applying or obtaining services from programs available to the resident.
- Provides consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs. (e.g. assisting staff to inform residents and those they designate about the resident's health status and health care choices and their ramifications).
- Evaluates staff according to facility Policies & Procedures updating job description/evaluations as necessary and appropriate.
- Maintains quality working relationship with the medical profession and other health related facilities and organizations.
- Coordinates social service activities with other departments as necessary.
- Works with the facility's consultants as necessary, and implements recommended changes as required.
- Delegates authority, responsibility, and accountability to other responsible department personnel and standardizing the methods in which work is accomplished.
- Interpret the department's policies and procedures to employees, residents, visitors, government agencies, etc.
- Reviews and develop a plan of correction for social service deficiencies noted during survey inspections as part of Facility Continuous Quality Improvement program.
- Actively participate in surveys (inspections) made by authorized agencies.
- Investigates and reviews departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken within 10 workdays.
- Interviews residents, or family members, as necessary, to obtain social history at the time of admission and as necessary in a private setting developing objectives and goals for the resident as appropriate.
- Assures that social service progress notes are informative and descriptive of the services provided and of the resident's response to the service.
What is required in this position?
- A Bachelors Degree in Social Work or a Bachelors Degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
- Must have, as a minimum, one (1) years experience as a Social Worker in a supervisory capacity in a hospital, skilled nursing care facility, or other related medical facility working directly with individuals.
- Membership in NASW is desirable.
- Registered as ACSW or ACBSW is preferred.
