The Avenue at Aurora is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment not commonly found in skilled nursing facilities. From our hotel-like environment to our all private suites , the Avenue is committed to providing an excellent level of physical, medical, emotional and spiritual support in an environment of comfort and respect. Our facility provides some of the area’s highest level of rehabilitation , nursing and medical support to individuals needing post-hospital care and services . We are currently seeking a part time Social Service Assistant with a passion for skilled and long term care. Under direction of the licensed social worker the Social Service Assistant is responsible for assisting the resident toward positive emotional and social adjustments in terms of their illness/condition, need for nursing home placement, and the placement itself. This position also advocates for the resident in regards to resident rights.
Responsibilities:
- Obtain pertinent social data about personal and family problems medically related to the resident's illness and care.
- Identifies individual social and emotional needs. Completes Social Service Assessment on admission, readmission, quarterly, and annually and with any significant change of condition.
- Assists in providing corrective action for the resident's needs by developing and maintaining individualized care plans.
- Maintains regular progress and follow-up notes indicating the resident's response to the plan and adjustment to the institutional setting.
- Assist with obtaining PASARR screens and level of care requirements for transfer, admission, or pay source change and up to date information about community agencies available for resident referrals.
- Assists with the completion of MDS
- Assist with the completion of admission paperwork and sign-in agreements
- Assists with the scheduling of ancillary in-house visits

Requirements:
- 2 years of college or 1-3 years of experience in a healthcare facility
- Self-starter
- Organized
- Strong communicator
- Ability to multitask
- Problem solving ability
- Extensive microsoft office knowlege/experience
- Knowledge of community resources
- Knowledge of Point Click Care is a plus
