We are searching for a high-level Social Services Professional for our team. Social Services is that integral part of our interdisciplinary team that effectively works with therapy, nursing, and the rest of the team to ensure the quality of the residents stay and then a smooth transition back into their home life.

The Social Service Director is responsible for monitoring the residents' mental and psychosocial needs and to provide the services to meet these needs to attain or maintain the highest practical level of physical, mental, and psychosocial well-being.
- Develop an ongoing care plan based on the residents’ needs, interests, and abilities as identified in the initial assessment.
- Participate in care plan meetings.
- Complete required record keeping upon resident admission, discharge, transfer; chart and document per facility policy.
- Implement healthcare directives & identify end of life care opportunities.
- Assesses the resident's potential for discharge as a member of an interdisciplinary team.
- Plans for the resident's discharge and post-discharge with the resident, family, and interdisciplinary team.
- Assists the resident and family in the implementation of the discharge plan.
Social Services Director must have the following:
- Bachelor’s degree in social work
- 2-3 years’ experience in social services/discharge planning in a long-term care/Skilled nursing setting
- Working knowledge of Medicare/Medicaid & Insurance
