Essential Functions of Social Services Supervisor:
- Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels
- Develop specific, measurable, realistic plans and objectives, which enhance social service daily
- Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their families
- Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors
- Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning. Include family in teaching as appropriate.
- Demonstrate knowledge of clinical social work and discharge planning. Interpret and promote residents’ rights and the “Residents’ Bill of Rights”
- Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
- Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
- Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and center policy is followed.
- Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
- Be flexible, organized, and function under stressful situations.
- Maintain a good rapport and working relationship with medical staff, departments, and employees.
- Manage and operate equipment safely and correctly.
- Treat patients and their families with respect and dignity.
- Support and maintain a culture of safety and quality.
Qualifications of Social Services Supervisor:
- Bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology
- One year of supervised social work experience in a health care setting working directly with individuals is preferred
- Current State Clinical Social Work Licensure
- Ability to work cooperatively as a member of a team
- Ability to communicate effectively with residents and their family members, and at all levels of the organization
- Ability to react decisively and quickly in emergency situations
- Ability to maintain confidentiality
- Knowledge of training techniques for clinical staff
- Candidates must maintain a working email address and phone number for employer communication.
