Georgia Pines Behavioral Health Crisis Center Programis currently seekingsomeone who has the passion, commitment, talent, and ability to work collaboratively with their peersto continue to advance the mission and vision of the organization.
The selected applicant will have the pleasure of providinga broad range of paraprofessional social services to patients/clients/consumers and their families. In addition, the outpatient discharge planner/case manager will have the following job responsibilities:
- Discharge connects need to be done daily on all discharges
- Speak with aftercare providers regarding client and their suicide risk status, must document this conversation. Make sure ROI is signed for any providers outside of GA Pines. Document if client refuses to sign ROI for aftercare provider
- Make sure to get discharge addresses, who will pick up client, and accurate phone numbers for clients being discharged.
- Make follow up phone calls on all clients, but especially high or moderate risk clients- first call must be within 24 hours after discharge. Document calls on SI well-being note.
- Talk with clients about their discharge options (i.e. SA residential, ACT, SA IOP, MAT program, individual counselling, etc). Make appropriate referrals as needed.
- Discuss barriers to discharge with clients. Maintain family involvement if client allows.
- Will assist with providing any resources to client that are available.
- Will assist inpatient case managers when needed.
- Will assist discharge assessor OP assessor when needed.
- May sit in on Treatment team daily.

Minimum Qualifications:
High school diploma or GED and two years' experience in a social service-relatedposition Or Bachelor's degree in a social service-relatedfield Or One year at the lower level or position equivalent. Position requires a valid driver's license.
