What it's like to be a Social Worker at Social Worker
As a Social Worker at Four Winds Manor, Inc., you will play a crucial role in the organization by overseeing all new admissions to our facility in Verona. Your responsibilities will extend to coordinating care specifically for short-term rehab residents, ensuring a smooth transition and continuity of care. This vital role involves facilitating care conferences with families and/or residents, attending weekly Medicare meetings, conducting necessary MDS assessments, creating individualized care plans, and coordinating discharge services as required. Your expertise and dedication will directly impact the well-being and comfort of our residents during their stay with us.
What matters most
To excel in the role of Social Worker at Four Winds Manor, Inc., candidates should possess a strong foundation in social work practices and principles. Ideal candidates will hold an active Wisconsin Social Work license (preferred) and a Bachelor's degree in social work or a related human services field. While at least 1 year of experience in a long-term care or healthcare setting is preferred, it is not essential. Successful candidates will demonstrate excellent communication skills, empathy, and the ability to work effectively within a multidisciplinary team. Proficiency in relevant software and tools, as well as a passion for providing high-quality care to residents, are key attributes that will contribute to success in this dynamic and rewarding position.
Knowledge and skills for the position are:
- Active Wisconsin Social Work license (preferred)
- Bachelor's degree in social work or other human services-related field (preferred)
- At least 1-year experience in long-term care or healthcare setting preferred though not necessary
