Job Title: Family Caregiver Support Program Assistant

Responsibilities:

• Serve as the primary screener for all inquiries related to the Family Caregiver Support Program.
• Complete client registrations and maintain accurate records.
• Assist clinical social work staff in outreach activities and community engagement.
• Provide information and resources to caregivers and families.
• Support program evaluation and reporting efforts.

Qualifications:

• High school diploma or equivalent; degree in social work or related field preferred.
• Strong communication and interpersonal skills.
• Experience in social services or healthcare settings is a plus.
• Ability to work collaboratively in a team environment.
