Responsible for providing psychosocial evaluations and treatment under the direction of a physician's plan of care to patients who are receiving home care services. Care to be provided under the supervision of a qualified MSW.

Responsibilities:

(25%)* Evaluates the patient's social, emotional, spiritual, financial, and environmental factors related to their illness, response to treatment, and adjustment to care.

 

(10%)* Develops a social work treatment plan based on assessment and input from patient, family, and other disciplines involved in care.

 

(25%)* Collaborates with physician and other team members regarding significant social and emotional factors that affect the medical plan of care. Advocates and negotiates with healthcare team and community agencies for patient/family to receive needed care.

 

(40%)* Provides social work interventions to patient and family including counseling, education, crisis management, patient?centered decision making, advance directives, care planning, resource linkage, funeral planning, etc.

Certifications and Licensures:

BLS - Basic Life Support

Required      Equivalent Experience - Required within one month of hire or transfer.

Required Education:

Education: BACHELORS DEGREE

 

Comments: Bachelors degree in Social Work or related field. Two years experience in a social agency or community organization dealing with health and/or welfare.

Either BSW or MSW will be considered for this position.
