Social Worker - Care Coordination - Richmond University Medical Center

The Social Worker provides social work services consisting of comprehensive case management, discharge planning, continuing care services and clinical social work services including crisis intervention under the direction of the department leadership. The ideal candidate will be responsible for the education of patients, families and/or significant others and provide community education as directed by the department or programmatic needs. In addition he/she will work as part of an interdisciplinary team, participate in departmental functions and represent the hospital and department as directed by department leadership in both on-site and off-site meetings and conferences.

Requirements:

-Graduate from an accredited school of social work.
-Current registration in New York State as an LMSW required.

-Experience with patient population required.

-Experience in a hospital or community health agency and additional language fluencies preferred.

-Computer literacy preferred.

Salary Range: $37.407/hr - $45.658/hr (Commensurate with Experience)
