Social Worker Hospital Case Management
We are seeking Social Workers for an Inpatient Care Manager role within a community based acute care hospital in Montgomery County, PA.
The Social Worker will be responsible for discharge planning, prior authorization for post-acute care and dialysis or homecare placement.
Job Details
- Weekly Estimated Pay: $1,900 - $2100 (plus travel stipend if 50+ miles away)
- Setting: Acute Care Hospital Case Management
- Start Date: 1/22/24
- Shift: Monday Friday 8am-4:30pm ; 40 hours per week
- Duration: 13 week contract
Job Requirements
- 2 years of hospital based social work experience
- Discharge Planning Experience
- MSW highly preferred, BSW required
